2011-2012 Star Systems competition entry form. Please use one entry form per routine. Entries must be postmarked 30 days prior to event. Please print legibly or type entry information and fill out form fully.

Contestant List Form

Print and duplicate. List each competitor performing one name per line. Please use one contestant list form per routine. Type or print legibly.

Credit Authorization Form

If paying by credit card, print and fill out the credit card authorization form. Please type or print legibly and make sure form is complete. A legible copy of the credit card being used (front & back) must be submitted with this form and faxed to 336-993-9075.
